$500 SIGN ON BONUS AFTER 90 DAYS!! The Case Manager will be responsible for providing direct support and assistance in case management to the residents participating in the program(s) and maintenance of residents daily records. The Case Manager will provide support to the Program Director. . This position will be responsible for performing all job duties and descriptions according to program policies and procedures. JOB FUNCTIONS • Ability to work with youth and/or their families in a changing setting which may include transitional care and aftercare outreach. • Coordinate (Campus) client acceptance: within 24 hours review referral packet and discuss with clinical team, set up interview and participate in interview with referral coordinator, discuss outcome with clinical team and communicate with collaterals regarding the acceptance of the client. • Coordinate (TLP) acceptance of client and transportation to the program. • Complete all intake paperwork within 8 hours of arriving (immediate needs assessment, have all releases signed, all med documentation in place, including MAR’s, orders, prescriptions, safety plan, signing placement agreement, mental health status form, review program policies and expectations and ensure collaterals have sent all of the clients documents. • Review safety plan and mental health status form with a clinician within 8 hours of intake. • Maintain all appropriate records according to Caring Together, EEC and Youth Services standards and guidelines. • Complete 72 hour treatment plan with an LPHA signature, ensure youth readiness assessment-YRAT is completed within 20 days of intake and singed by DCF & Client. Complete Bio-Phsyc Social within 30 days of intake-obtain LPHA signature, Complete initial 30 day treatment plan with LPHA signature (by 30 day) • Coordinate treatment team reviews, complete updates to treatment plan and obtain signatures. Required every 60 days ( 90-120 etc) • Complete CANS (child assessment needs and strengths) assessment within 30 days. • Update Safety Plans every 30 days or as needed. (use as documentation training with staff). • Update YRAT every 6 months.( utilize staff to support completion) • Complete Risk Assessment as needed. • ** (program specific)Case Manger is required to learn and use Virtual Gateway to document treatment plans, treatment plan reviews, and incident reports. • Required to utilize language in accordance to rehab options expectations. • Responsible for the client binder ensure case index and subsequent documents are signed and in order. • Participate in residents’ team meetings with collaterals • Coordinate and record all residents appointments and daily schedule • Oversee staff to ensure documentation and completion of client progress notes and incident reports, ensure rehab options language is used in progress notes, collaborative problem solving language for incident reports. • Collaborate and communicate updates and important information with DCF, school personnel and any other provider agencies regarding the residents’ services and treatment goals ( weekly brief emailed summary of progress towards goals to collateral team) • Assure that residents’ quality of life and standards are maintained including: dietary, hygiene, appropriate clothing and building cleanliness. • Participate in program support, including participating in meal preparation and ensuring cleanliness and sanitation of the program as needed. • In collaboration with direct care staff ensure groups focusing on harm reduction and increasing independent living skills are scheduled as required. • Responsible for providing leadership and act as a positive role model for residents and staff • Required to use personal vehicle, when Centerboard vehicle is not available in order to meet appointment and/or aftercare outreach requirements. • Responsible for reading communication log and providing client or program updates in the log as needed. • On Call - Participate in the On Call rotation and document calls in Google Sheets • Complete all Assessments/forms in accordance with State/EEC/DCF regulations- i.e. Program Assessment, YRAT, CANS, Substance Abuse Evaluation, Mental Status Exams, Treatment Plan, Biopsyc, Discharge/Termination forms, Safety Plans, and use of the Virtual Gateway. • Collect Program Fees bi-weekly and submit to Fiscal- (in teen parenting programs only) and chart individual payment schedules, payments and balances. • Conduct In- House trainings for staff- i.e Safe Sleep Tr., IR documentation refresher, BMT refresher, COVID testing, Progress and Advocate Note documentation, Staff shift responsibilities refresher and others. • Create Groups for the Program-(in teen parenting programs) mandatory to offer PAYA and parenting groups. (In all programs)- Budgeting Group • House Meetings – Conduct a house meeting for the residents and keep the agenda and minutes in the Staff/House meeting Binder • Trainings - Keep a log of training hours and participate in trainings through Track Trainings, in person/on the phone/online. • Budget – Monitor resident monthly budgets and record budget information on Filemaker. • Discharge/Transition planning - Oversee all communication regarding residents and their teams, in order to keep next steps in mind. Discuss the program expectations in order to transition a youth into the program. When issues arise, discuss all matters with the DCF team and the PD and participate in all team meetings. • Interview/Intakes with residents – (adolescent programs) Interview new residents and talk with DCF teams for possible placement. Intake new residents in the presence of the DCF SW. The case managers should present the Program and the Resident Handbook at that time and the CM will complete all intake paperwork with the youth and the DCF SW. Medication will need to have accompanied Doctor’s orders. The resident is expected to move in after the intake process is complete. • Interview New Employee Candidates/Interns – Interview new candidates for hire and interns via FaceTime/zoom/ in person. Complete all HR interview questions. Scan and email HR background paperwork to the candidate/intern and ask them to fill them out and email them back to me along with a copy of their Driver’s License. After the interview make a recommendation to accept the individual or not and email everything to the HR department for their review, or drop off the paperwork at the main office in any of the HR mailboxes. • Transportation for youth under 18 years old – Staff must transport and stay with a youth for the following appointments: Hospital/ER visits, collateral meetings, court, school meetings, doctor and dentist appointments. • CQI, maintain, track, document, and create information necessary for the individual program. Attend meetings and present information as required. (CQI, Theory of Change ) • Perform other duties, responsibilities and related tasks as requested. QUALIFICATIONS Bachelor’s degree in psychology or related field and at least two years of experience working in a residential setting. Case Manager’s must have the ability to work flexible hours to meet the needs of clients schedules. Required to pass initial competency evaluation as well as continued assessments of professional development and future trainings. The Case Manger must possess the ability to use collaborative and proactive solutions during behavior management. Qualified candidates must possess strong communication and organization skills and be a willing participant in Centerboard’s multicultural awareness, diversity, equity and inclusion trainings, meetings and philosophy.
